Our History

Case Management of Michigan, Inc. grew out of the need to successfully assist consumers to transition from state institutions to more inclusive community placements.  Since 1995 we have been the industry leader in providing cost effective and flexible case management services that have reduced the cost of care.  Traditionally we work with seriously mentally ill adults, including individuals with a long history of psychiatric hospitalizations and/or multiple unsuccessful community placements.

Case Management of Michigan works closely with adult foster care providers to carefully evaluate each case and create an individualized plan for success with residential placements. Small caseloads and intensive case management services allow us to be an active force in our consumers’ lives. Most of our consumers are enjoying their longest community placement. We believe that reducing the cost of care can be achieved while improving the quality of life for our consumers.

 

 

 

Our Services

Hospital Liaison Services

Allow the professionals at Case Management of Michigan to monitor and suggest placement options to assist consumers to live more independently.  Utilizing standard tools including the LOCUS and Multinomah Community Ability Scale, Case Management of Michigan, Inc. can act as the consumer's advocate to encourage an expedient placement in the community.  

Assessment

The first step to a successful placement is a comprehensive assessment. Case Management of Michigan, Inc. identifies the least restrictive AFC setting. We then meet with the potential consumer, AFC provider, hospital, and Community Mental Health staff to determine the most appropriate services.

Placement

Once the appropriate facility has been identified, Case Management of Michigan, Inc. will coordinate transportation, benefits, and the Person-Centered Planning process. The assigned case manager will meet with the new consumer within twenty-four hours to implement an interim treatment plan and begin the PCP process. In addition to the traditional professional assessments, we also elicit goals from the consumer, guardian, and any other appropriate parties.

Ongoing Care

Our commitment to our clients is demonstrated by weekly case management contacts and frequent community outings. Case management contacts are designed to support the individual and encourage positive problem solving behaviors and adaptive functioning. We work collaboratively with Community Mental Health  Liaisons to develop unique approaches to better assist consumers through the continuum of care. Case Management of Michigan, Inc. also provides routine progress reports, 24 hour on call services, recipient rights services, and coordinates psychiatric medication reviews.

Our average cost of service each month is less that one day of hospital care!

 

Location

Our main office is located in the historic State Theatre Building on Kalamazoo, Michigan.   

The State Theatre has been a fixture in downtown Kalamazoo since 1927. The original founder of the theatre was Colonel William Butterfield. John Eberson was the renowned architect who designed the theatre. In line with theatre design conventions of the era, the idea was to create a unique atmospheric effect. The early roots of the theatre were found in the Vaudevillian tradition of performance. Many acts that frequented the State Theatre included opera, dramas, big bands, ballet, dance reviews, stage shows and movies.
